Mained free from neurologic events. Although this difference did not reach statistical significance, probably owing to the small sample size, and despite the fact that a few patients either asymptomatic or with associated carotid plaques were also included, the study supports the surgical indication of symptomatic kinking of the ICA. We share this convinction, and deem that a crucial point is the evidence that kinking is the cause of preoperative neurologic symptoms. This evidence is usually apparent when dealing with hemispheric TIAs and strokes, but it may be not equally sound when nonhemispheric symptoms are involved. In the present series, more than 40 75% ; of the 54 patients were operated on for true hemispheric symptoms, whereas one fourth of the whole study population was operated on for nonhemispheric symptoms alone. In this latter group, criteria for inclusion in the study were rather strict and always validated by a neurologist. One could argue that, overall, 12 22% ; of the 54 patients had diabetes mellitus and that 33 patients 61% ; were treated for arterial hypertension; both of these conditions may mimic a nonhemispheric neurologic syndrome. However, their incidence was comparable to that of any series of patients with vascular conditions, and in all of the cases these conditions were well controlled by medical treatment. Finally, 49 91% ; of the 54 patients, and all but 1 of those with nonhemispheric symptoms, were relieved by revascularization of the ICA. When nonhemispheric symptoms are involved, an association between ICA kinking and significant stenoses of the VAs would be expected. This was not the case in the present study: all of the lesions consisted of hypoplasias of a single VA, which did not require surgical treatment, whereas no stenotic or embolizing lesions of a dominant VA were present. Furthermore, none of the patients either had a revascularization of a VA before revascularization of the ICA, or required a subsequent VA operation. When a significant or embolizing stenosis of a dominating VA is associated with a significant kinking and or stenosis of the ICA in a patient with cerebral nonhemispheric symptoms, the ICA should be revascularized first. However, as revascularization of the ICA alone may be followed by a recurrence of symptoms, both ICA and VA should be simultaneously revascularized if their lesions are ipsilateral.34 If the stenosed, dominating VA is contralateral to the diseased ICA, we would revascularize the ICA first, and would operate on the VA later on, only if symptoms persist. These conditions did not occur in any of the patients of the present series. The restenosis rate of 9% 5 patients ; in this series is comparable to that experienced in other studies.12 No statistical correlation between the technique of revascularization and recurrence rate could be established. Restenoses were approached with the same policy deserved to those occurring after standard carotid endarterectomy: in the absence of symptoms, of preocclusive narrowing, and of contralateral carotid occlusion, they are controlled with duplex ultrasound study twice a year, but none has been considered yet either for reoperation or angioplasty and stenting. For the single patient who experienced recurrence of preoperative, nonhemispheric symptoms, despite a patent revascularization, no satisfactory explanation could, because piracetam com.
Establishing a diagnosis Depression is often difficult to diagnose in general practice, especially when it is associated with physical symptoms.5 Diagnostic tools are available and include the fourth revision of the American Psychiatric Association's Diagnostic and Statistical Manual DSM-IV ; 6 and the tenth revision of the International Classification of Diseases ICD-10 ; 7 criteria. Table 1 shows DSM-IV criteria for major depression. Patients who do not fit these criteria may have dysthymia, a chronic depressive state 2 years duration ; , or minor depression.6, 8.

BONT E PREVENTS SEIZURE-INDUCED ACTIVATION OF CASPASE 3 IN RAT HIPPOCAMPUS Ilaria Manno1, Flavia Antonucci2, Matteo Caleo2, Yuri Bozzi2 2 Laboratory of Neurobiology, Scuola Normale Superiore, Pisa; CNR Institute of Neuroscience, Pisa. Temporal lobe epilepsy TLE ; is one of the most common forms of human epilepsy, characterized by recurrent seizures, which are often resistant to pharmacological treatment. Neuropathological and neuroimaging studies have also shown a clear link between TLE and brain damage. It is calculated that about 70% of drug-resistant TLE patients present temporal lobe damage. Cell loss within Ammon's horn in the hippocampus is the most commonly observed lesion, although other regions may be affected. Experimental studies performed on several animal models of epilepsy allowed to confirm that a prolonged seizure or status epilepticus SE ; can cause neuronal death in the brain. Induction of apoptotic pathways has been shown to contribute significantly to cell death after seizures. We recently demonstrated that delivery of Botulinum Neurotoxin E BoNT E ; to the rat hippocampus markedly reduces the occurrence of kainic acid KA ; -induced seizures. BoNT E treatment also prevents neuronal loss associated with KA seizures. It is conceivable to assume that BoNT Emediated neuroprotection of hippocampal neurones involves the blockade of apoptotic pathways. In our study, we sought to elucidate the molecular mechanisms through which BoNT E prevents the death of hippocampal neurones. To this purpose, we evaluated the expression of two proteins involved in the activation of apoptotic pathways after seizures: c-Jun and caspase 3. The induction of the transcription factor c-Jun and its subsequent phosphorylation by the Jun N-terminal kinase JNK ; are among the first steps of apoptotic cascades triggered by KA seizures. Cleavage of procaspase 3 by caspase 9 determines the activation of the apoptosis effector caspase 3, and occurs in the rodent brain as a consequence of experimentally induced seizures. Our results show that intrahippocampal delivery of BoNT E reduces both c-Jun phosphorylation and cleavage of caspase 3, which occur following KA-induced seizures and premphase.

3-Adrenergic Crosstalk with JAK STAT Signaling Downregulates Expression of Angiotensin II. Soren Westphal * 1, Nina Perwitz2, Volker Ott2, Daniel Kraus2, Rafael R Schick1, Mathias Fasshauer3, Johannes Klein2. 1Dept of Intern Med, Fed Armed Forces Hosp, Ulm, Baden-Wuerttemberg, Germany; 2Dept of Intern Med I, Univ of Luebeck, Luebeck, Schleswig-Holstein, Germany; 3Dept of Intern Med III, Univ of Leipzig, Leipzig, Sachsen, Germany. Sympathetic nervous system activity is implicated in dysregulated energy homeostasis and hypertension. Angiotensin II ATII ; is a key regulator of vascular tone. Adipose tissue is densely innvervated, expresses ATII, and plays a critical role in mediating cardiovascular morbidity. We investigated adrenergic regulation of ATII expression in an adipocyte model of SV40T-immortalized brown adipocytes. Treatment of differentiated adipocytes with a 3adrenoceptor agonist CL316243 ; significantly reduced ATII gene expression as assessed by real-time polymerase chain reaction. This effect was time- and dose-dependent with a maximal 60 % inhibition after 8 h of treatment p 0.01 ; . Janus kinase Signal transducers and activators of transcription JAK STAT ; signaling is important for mediating inflammatory stimuli and has been implicated in the pathogenesis of the metabolic syndrome. Acute 3adrenergic stimulation time- and dose-dependently increased phosphorylation of STAT1 and STAT3 as assessed by immunoblotting with phospho-specific antibodies. Exposure of differentiated cells to the 3-selective agonist 100 nM ; for 20 minutes induced a four-fold increase p 0.01 ; . These effects were abolished by inhibition of protein kinase A or JAK2 using the pharmacologic inhibitors H89 and AG490, respectively. By contrast, inhibition of SRC kinase using PP2, phosphatidylinositol 3-kinase PI 3-kinase ; using LY294002, and protein kinase C PKC ; using GF109203X did not affect 3-adrenergic stimulation of STAT1 or STAT3, thus indicating a PKA- and JAK2dependent, but PI 3-kinase-, PKC-, and non-receptor tyrosine kinases-independent pathway. On the level of transcriptional regulation, inhibition of PKA or JAK2 alone did not affect the negative 3-adrenergic regulation of ATII expression. However, combined inhibition of PKA and JAK STAT pathways significantly reversed the inhibitory 3-adrenergic effect p 0.05 ; . Taken together, our findings demonstrate sympatho-adrenergic crosstalk with JAK STAT signaling which contributes to the regulation of components of the renin-angiotensin system in adipose tissue. This crosstalk may represent a molecular link between obesity and hypertension, two key components of the metabolic syndrome. Gunasekaran, Faculty of Engineering Technology, Multimedia University, 75450 Melaka, Malaysia] - NEUROCOMPUTING 2003 55 3-4 ; - summ in ENGL The Back Propagation Network BPN ; is one of the most widely used neural networks. It has a distinct training phase and then it is put to use. It is observed that certain training sets would get trained and certain others would not get trained in a BPN. This is an inherent property of the training set and the BPN architecture. This paper proposes a new training index that may be evaluated after a certain number of training epochs and would indicate the ability of the BPN to train for the existing topology and training set. The index is evaluated for several logic gates and the results are presented. 2003 Elsevier B.V. All rights reserved. 641. Topological local principal component analysis - Liu Z.-Y. and Xu L. [Z.-Y. In this paper, we drop a number of customary features that are used in neural modeling to improve performance yet are able to predict daily returns for the four main exchange rates with accuracy above levels previously reported for comparable data sets. Significant outperformance relative to a random walk without drift and against the mean value predictor is reported, indicating that the transparent neural network used in the study is finding new and potentially profitable information. 2003 Elsevier B.V. Effects. These drugs mostly nonprescription ; and nutrients are featured in the popular press as memory- or cognitive-enhancing supplements, and are recommended as part of treatment regimens at some aging clinics. They include Ginkgo biloba. phosphatidylserine PS ; , vinpocetine, acetyl-L-carnitine ALC ; , piracetam, choline-related nutrients thought to be involved in producing acetylcholine ACh ; , and antioxidant agents like vitamin E. These are often combined into memory-cocktail supplements and sold commercially. For example, the first four nutrients listed have recently been combined into a single cocktail supplement and sold as Memory 2000 produced by Natural Balance ; . The Aging Brain The presumed neural benefits of these nootropic agents may articulate well with the neural declines associated with normal aging and with degenerative neural pathologies commonly seen in older adults. The growing evidence suggests at least three prominent global changes in the brain that occur with age. First, the neurons show multiple changes, and neuronal changes are a more decisive hallmark of age than widespread death of neurons.5 Briefly, the aging-related neuronal changes include accumulation of nonessential substances e.g., yellowish brown lipid lipofuscin--"wear and tear" pigment ; , loss of essential myelin fatty material around axons; the axon conducts an electrical signal away from the neuron body, and myelin promotes speedy and reliable propagation of the signal ; , and general shrinkage. With regard to age-related changes in memory and cognitive functioning, it is perhaps significant that lipid lipofuscin accumulates prominently in cortical neurons, 5 and myelin loss is most notable in the association and limbic cortices specific areas of the cerebral cortex ; .6 Second, the connections between neurons, not just the neurons themselves, change with age. There is a reduction in the branching of dendrites fibers on which axons of other neurons terminate ; and a decline in the number of properly functioning connections between neurons.5 Aging may depress the availability of neurotransmitters such as ACh, and ACh seems to be heavily involved in neuron networks associated with memory. Third, with age the cerebrovascular system shows numerous structural changes, diminishing cerebral blood flow, and declining cerebral blood volume. With extreme shortage or suppression of blood flow, a condition called ischemia exists and provera.

Memory enhancing drugs is a broad field that has developed since the 1970's, although it has been known for a long time that stimulants can improve learning performance39. Today we have several families of memory enhancing drugs affecting different aspects of the learning and encoding process. They range from stimulants40, nutrients41 and hormones42 over cholinergic agonists43 and the pi4acetam family44 to ampakines45 and consolidation enhancers46. It should be noticed that learning enhancement or modification might not just be useful for traditional memory but also for unlearning phobias and addictions47, potentially allowing memory modification. The earliest cognition enhancing drugs were mainly nonspecific stimulants and nutrients48. Glucose is the major energy source for the brain, which cannot store it and relies on a continuous supply to function. Increases in availability either due to ingestion or short-term stress hormones ; improve memory49. Stimulants enhance memory either by increasing the amount of neuron activity or by releasing neuromodulators, both factors that make the synaptic changes underlying learning more likely. The growing understanding of memory allowed the development of more specific drugs. Stimulating the cholinergic system, which appears to gate attention and memory encoding, was a second step. Current interest is focused on intervening into the process of permanent encoding in the synapses, which has been elucidated to a great extent and hence has become a promising target for drug development. The goal would be drugs that not just allow the brain to learn quickly but also facilitate selective retention of the information that has been learned. It is known that the above families of drugs can improve performance in particular memory tests. It is not yet known whether they also promote useful learning in real-life situations and ramipril.

PHAM has special historical and functional importance. As noted by Obeso et al. 40 ; , action myoclonus is the most disabling form of myoclonus, and the single most common cause of action myoclonus is the LanceAdams syndrome. Some of these patients improve spontaneously over time. Most have had reduced CSF levels of 5-HIAA, the serotonin metabolite, when CSF studies have been done. Perhaps half of the patients taking 5-HTP experience significant benefit. The CSF metabolite data are crude predictors of the 5-HTP response of individual patients, those with normal 5-HIAA values being less likely to respond. BDZs are the most useful drugs for patients unresponsive to 5-HTP. Many patients respond to several drugs including 5-HTP, BDZs, and valproic acid. Drugs such as baclofen, piracetam, and primidone have been helpful in scattered cases. Some patients do best with a combination of two or more drugs 40 ; . Clonazepam has significant effects on 5-HT turnover, and its suppressant effect on DDT myoclonus in mice is antagonized by several 5-HT receptor blockers 41 ; . Hence, clonazepam suppression of myoclonus was initially though to involve 5-HT receptors. However, several other BDZs are effective for PHAM, and BDZ therapy of human myoclonus is probably best explained by effects at GABAA-BDZ receptors with secondary 5-HT effects, as discussed in section VII. All patients with PHAM have brain-stem pathology, although some meet the criteria for cortical reflex myoclonus and have more than one myoclonic generator. Many have relatively good cognitive function. This group of patients seems more likely to respond to 5-HTP than any other. Those unable to tolerate 5-HTP may be helped by other serotonergic drugs including trazodone and fluoxetine. V. ANIMAL RESPONSE Myoclonus often consists of relatively symmetrical jerks, sometimes limited to the head and neck or even to eye blinks. One explanation for these observations is MODELS AND THE STARTLE and retin-a and piracetam.

E. coli 0157: H7 is one of hundreds of strains of the bacterium Escherichia coli. Most strains are harmless and live in the intestines of humans and animals, but this strain produces a powerful toxin, which can cause severe illness. It is a strain of verocytotoxin producing Escherichia coli VTEC ; known as E. coli 0157 and is found in the intestines of some cattle and other domesticated animals such as goats, also in the intestines of infected people. What are the symptoms of infection with E. coli 0157? E. coli 0157 is often very mild, but some people develop diarrhoea, which can be severe and bloody, with abdominal cramps. A few cases especially in children under 5 years of age and older people ; may develop a complication called haemolytic uraemic syndrome, which is a form of kidney failure. They may need admitting to hospital for renal dialysis or blood transfusion. How is E. coli 0157 spread? There are 3 main ways in which the infection can be spread to humans. Firstly, the bacteria are present in the faeces of some farm animals and this can contaminate the carcass during slaughter. E.coli 0157 present on the surface of meat can become mixed into the meat during mincing. The bacteria present in faeces may also contaminate udders and milking equipment and get into the raw milk. Secondly, the infection can be acquired during visits to farms and fields where farm animals live. Their faeces can be found in the general environment e.g. gates and fences ; or contaminate salads and vegetables being grown in the vicinity. The faeces may also be picked up on shoes, clothing and fingers. Infection can develop if the bacteria are able to get into the mouth through poor hygiene or eating poorly washed salads and vegetables. Thirdly, the infection can be passed from person to person by direct or indirect contact with the faeces of people with E. coli O157 infection. This may happen within families and households, or nurseries etc during nappy changing, or using toilets, or toys that may become contaminated with faeces from socks or shoes e.g. bouncy castles, ball pools, paddling pools etc. Ingestion of a small number of organisms can cause illness. How can the spread of E. coli 0157 be prevented? Don't eat undercooked meat products, e.g. beef burgers and minced beef Thoroughly cook meat until the juices run clear Drink only pasteurised milk Wash hands before handling food, after using the toilet or changing nappies Wash animal faeces from shoes and clothing, followed by hand washing Follow recommended precautions for school visits to farms Part Four 11.0 ; In residential care, nurse in a single room with en-suite, or dedicated, toilet until diarrhoea has stopped for 48 hours may need negative stools see below and rimonabant.

Human Cloning and Human Dignity: An Ethical Inquiry, a July 2002 report by the President's Council on Bioethics, considers the advisability of human cloning, both for the purpose of producing children and for biomedical research. The report examines cloning from a human, technological, and ethical perspective, rather than merely as "an isolated technical development, " in the wake of major reports on this subject by the National Bioethics Advisory Commission in 1997 and the National Academy of Sciences earlier this year. Commission members determine that cloning to produce children would "violate the principles of the ethics of human research, " given that "high rates of morbidity and mortality in the cloning of other mammals" indicate that cloning human embryos "would be extremely unsafe." In addition, such cloning would be unethical because "cloned children may experience serious problems of identity" and "might come to be considered more like products of a designed and manufactured process than `gifts' " and because such practices may facilitate the possibility of "cloning to perpetuate genetically engineered enhancements." Commission members further find that cloning for purposes of biomedical research is potentially problematic because "it ignores the hazardous moral precedent that the routinized creation, use, and destruction of nascent human life would establish, " because it crosses conventional moral boundaries in the "creation of human life expressly and exclusively for the purpose of its use in research, " and because "giving moral approval to such research risks significant moral harm to our society." The majority of study authors, accordingly, recommend that policymakers institute a permanent ban on cloning to produce children and a four-year moratorium on cloning for biomedical research. Free copies of this 194-page report are available on the Web at bioethics.gov cloningreport.

Adverse events were infrequent and similar in both treatment arms Table 4 ; . There were no treatment-related deaths. QOL Compliance with the QOL portion of the study was high. The PPI and PROSQOLI scales were obtained in 100% of initial clinic visits and did not differ between the study arms over subsequent cycles of therapy. The incidence rates of QOL responses from all patients with a baseline assessment were 39 37.5% ; of 104 patients on the clodronate arm and 44 42% ; of 105 patients on placebo P .61 ; . Median changes from baseline in the PROSQOLI scores for all patients who had completed a minimum of two assessments are illustrated in Figure 3. The distribution favored the clodronate arm for the pain domain P .022 by Wilcoxon rank sum test ; . There were no significant differences in median changes between the arms in the other domains. Similarly, on analysis of the maximum changes in PROSQOLI scores, the distribution favored the clodronate arm P .042, by Wilcoxon rank sum test ; , with no significant differences seen in the other domains. Biochemical Markers of Bone Resorption Urinary calcium, creatinine, and pyridinium cross-links were assessed at 12 and 24 weeks after randomization. A logistical regression model was used to explore changes from baseline as potential predictors of palliative response. For each marker, change from baseline of the biochemical marker as fixed effect was fitted to the longitudinal data. A model with changes of all three markers was also fitted to the data.
